The REHAB Physical Therapist Responsibilities:
Coordinate patient care with nursing, OT, PT, SLP, and family to achieve safe discharge home or placement to next level of care. Provide skilled physical therapy services/interventions in accordance with physician orders. Assesses patient needs, plan for, evaluate, and modify care to meet goals of physical therapy interventions. Evaluate, record and report on patient's response to treatment and progress toward identified goals. Integrate physical therapy treatments with other aspects of patient care. Assist patients to reach their maximal reasonably levels of function and to cope with their limitations. Monitor, record and communicate patient condition as appropriate utilizing computerized documentation systems. Instruct and educate patients and families. Participate in patient discharge planning. Provide age and culturally appropriate care.

MENTORSHIP for a New Grad with a staff of experienced Therapists!

Education, training, experience:
Completion of an accredited Physical Therapy program Eligible for PT Licensure in the state of Georgia New Grads welcome to apply
