Responding to customer and community needs regarding community education. Essential Functions/Responsibilities · Coordinates with the Executive Director and Director of Patient Care Services regarding the planning, implementation and evaluation of in-service and continuing education programs. · Minimum of one year experience as a professional nurse within the last three (3) years or have a baccalaureate degree in nursing from a program accredited by the National League of Nursing. · Promote hospice in the community and ensure the educational needs of the community · Maintains comprehensive working knowledge in the field of community educational and Conducts community education · Facilitates the hospice referral process and serves as a resource to review and consult available patient information related to the case · Coordinates and communicates with hospice agency leadership and referral sources to promote care coordination and a timely admission process.
- Confers with Medical Director or Attending Physician regarding patient hospice determination criteria. · Obtains physician orders and Certification of Terminal Illness, as well as other admission documentation as required. · Completes hospice consults/evaluations, admissions and initial assessment of patient and family. · Promptly responds to customer and referral source requests and concerns · Quality Assurance Performance Improvement (QAPI) process.
- Participates in IDT meetings and works with the IDT to determine the patient/family plan of care as needed. Position Qualifications · Graduate of an accredited school of nursing. · Current RN license in the state, · CPR certification · Valid driver's license · Minimum of one year experience as a professional nurse within the last three (3) years or have a baccalaureate degree in nursing from a program accredited by the National League of Nursing. The Ideal candidate should have worked with any hospice company.
