The Disease Management RN uses educational and motivational techniques to empower participants with chronic diseases to more effectively manage their disorder. By working collaboratively with the participants and their providers, this individual will work to decrease actual and potential disease complications while increasing the participant?s sense of control and efficacy.

Specifically, the Disease Management RN performs the following:
Performs on-! site and telephone disease management assessments and follow-u! p interventions.
Develops participant educational plans to promote self-management.
Provides direct coaching to participants on the pathophysiology of their disorder and implications (including psychosocial) of same.
Coordinates activities and provides hard copy and electronic resources to address the educational needs of the participants.
Collaborates with the participants to identify and overcome obstacles and barriers to a healthier lifestyle.
Works with participants to explore the various treatments available and their personal role and responsibility in maximizing the wellness resources available.
Communicates and coordinates on-going health care services with providers.
Implements strategies to ensure participants follow up on agreed upon activities and appointments.
Develops and implements evaluation criteria to assess the success of the program on both a micro and macro level.
Travel up to 30percent of the time between the Health ! System campuses.
Performs other duties as assigned.
