Jpb Summary: This is a liaison position requiring clinical experience, self direction, attention to detail and follow-up, and ability to communicate effectively with staff, patients, families and the public. In conjunction with physicians and members of the Disease Site Tea! ms, the Nurse Navigator is responsible for guidance and coordi! nation of services for oncology patients and their caregivers through the continuum of care. Required Minimum Education: Bachelor of Nursing degree required. Masters of Science degree in Nursing preferred. Required Minimum Experience: 3 or more years of adult acute care clinical experience. Oncology experience preferred. Discharge planning or case management acceptable. Required Minimum Skills: Self directed and able to organize and complete several tasks / projects simultaneously. Extensive understanding of oncology patient care. Proficient in review, analysis and maintenance of medical records and other patient care information. Demonstrated skills in verbal and written communication. Ability to communicate in a culturally diverse environment. Ability to maintain strict confidentiality.
