Experience Required: At least 3 years

Position Description:
BASIC FUNCTION:
This position is responsible in a clinical care advisory role to assess members for wellness education and disease management; introduce members to website tools to better manage their health; educate members regarding wellness and specific conditions; facilitate the coordination of care for members identified through claims; self-referral/referral from the UM

and/or CM departments, or physician referral.

JOB REQUIREMENTS:
1) Current Registered Nurse with unrestricted license in the state of operations;

2) 3 years clinical experience;

3) Managed Care experience;

4) Patient education experience;

5) Computer/system software experience including Web;

6) Verbal and written communication skills; analytical skills; sound clinical judgment.

PREFERRED JOB REQUIREMENTS:
1) Disease Management experience;

2) Bilingual in Spanish.
