The primary function of the Physical Therapist is to provide direct care, patient education and the management of physical therapy called for in the pat! ient’s treatment plan. Consult with and educates other care team members concerning applicable patient assessment and treatment protocols. Serve as a rehabilitation resource for the care team.

• Coweta & Fayette Counties
• Douglas County

EDUCATIONAL REQUIREMENTS: Graduate of an accredited School of Physical Therapy with a Baccalaureate or Master’s Degree.

LICENSURE: Must have a current licensure by the Georgia State Board of the specified therapy.

EXPERIENCE REQUIREMENTS: 2 years in home health who meets the standards for coordination of care and documentation or 5 years clinical experience but not in home health. Preference given to home health experience. Previous work in Rehab Centers or Sub-acute units are a plus.

JOB KNOWLEDGE: A professional knowledge of the theories, principles, practices and techniques inherent to the scope of pr! actice. Communication skills to interact effectively with pati! ent, family, care team members, referrals and community resources. Knowledge of accrediting bodies’ standards, Medicare, Medicaid and other funding source regulations as applied to home care services. Demonstrates competent techniques and practices of their profession.

COMPUTER SKILLS: Must be familiar with general use and functions of the computer, such as, user names and password concepts; internet; e-mail; navigation of computer desktop or laptop, including starting programs, using files, and windows, effectively use navigation buttons and tool bars; ability to self-manage online HR services and online training programs.

TRANSPORTATION: Must have valid automobile insurance and reliable transportation.
