WELCOME to United Hospice of Atlanta, part of the UHS-Pruitt family of companies, now has! an open position for a RN Admission Nurse.Job Description: Screen referral for appropriateness and perform admission to facilitate the transition of the patient and family into the hospice program.Licensure/Certification/Education Requirements: Currently licensed as an RN in the states where you will provide care. Must have current driver's license and valid automobile liability insurance. Must have certification in basic life support CPR. A minimum of three (3) years nursing experience with hospice or home care preferred, and has the ability to work a flexible schedule.
Other Training, Skills, and Experience Requirements: Ability to work effectively independently and within an interdisciplinary team orientation to United Hospice and any other training that may be necessary to complete the assigned duties effectively. Primary responsibility for taking referrals to hospice and home care programs and providing follow-up on those "Not Taken Under Care." Makes ! field visits to evaluate appropriateness, determine eligibilit! y for services, and present hospice or home care program. Ability to work a flexible schedule. Insures communication of assessment and reassessment findings to attending physician.
