The Licensed Practical Nurse (LPN), Long-Term Care :
Under the supervision of a Registered Nurse, provides patient care using the nursing process of assessment, planning, implementing, and evaluation Uses good judgment Utilizes patient care supplies/equipment in a fiscally responsible manner Exhibits positive interpersonal skills The LPN, Long-Term Care performs the following essential responsibilities:
Effectively manages the care of assigned patients using knowledge, clinical skills, judgment and decision making skills for the specific area, under the direction of a Registered Nurse Delivers patient care utilizing guidelines of care and appropriate resources, including personnel, services, supplies and equipment Reviews, evaluates and takes appropriate action regarding results of diagnostic tests Administers medications according to SGHS policy Assists the RN in performing the nursing assessment to identify and prioritize physical psychosocial an! d cultural needs in order to develop a plan of care Evaluates ! and reassesses nursing interventions based on interdisciplinary plan of care and makes revisions as appropriate Assists with the development and implementation of a patient/family education plan Documentation meets compliance with regulatory standards
The ideal LPN, Long-Term Care candidate should possess the following minimum qualifications:
Current Georgia LPN license
Nursing skills as defined in the law governing the practice of nursing in Georgia
Current American Heart Association CPR Certification (BCLS) preferred
Long-term care experience preferred
