Reports to and is supported by the ARHH Clinical Management Team. Provides skilled nursing comprehensive assessments; physician order interventions in compliance with professional knowledge and skills in accordance with accepted standards of clinical practice. Coordinates and facilitates a holistic approach in the provision of ongoing assessments, problem identification and resolution, specific therapeutic interventions, as well as, patient/family education and training activities to achieve individualized patient/client goals and outcomes. Provides direct patient care, primarily for patients, adolescent throu! gh geriatric.
Requirements:
- Assoc! iate’s Degree
- 1+ years of experience of medical-surgical nursing or area of specialty
- Basic understanding of Medicare home health regulations and knowledge of home care reimbursement issues
- Current Georgia RN license and valid Georgia drivers license
- Strong verbal and communication, critical thinking and decision making skills
- Creativity, initiative, flexibility and the ability to work independently, as well as personal maturity and the capacity for objective decision-making and problem resolution
- Ability to learn computer applications quickly
