Bone Marrow Transplant Discharge Coordinator

Description: Odell Medical Search is currently seeking to employ Bone Marrow Transplant Discharge Coordinator right now, this job will be placed in Georgia. Cancer Center in the Atlanta, GA area is seeking a Discharge Coordinator for their outpatient Bone Marrow Transplant services.
Contributes to excellence in patient/family centered care by coordinating the health care of select patient populations/designated services across the continuum of care using the nursing process.
Coordinating the patient's discharge from the hospital to the out-patient clinic.
Develop and maintain the plan of care.
Ensures that all discharge needs a! re met.
Schedules all discharge appointments, treatme! nts, and protocol specific orders.
Completes needed discharge follow up phone calls to patients and documents appropriately.
Qualifications:
BSN required
3 years of clinical nursing experience as a Registered Nurse with at least 2 years of recent experience (within the last 5 years) in Bone Marrow Transplant or Oncology
OCN certification preferred
Discharge Coordinator experience preferred
