This position is responsible for:
General Summary:
The primary role of the Bariatric Coordinator is coordination of care for bariatric surgery patients and management of ongoing Bariatric Surgery Center of Excellence (BSCOE) compliance. This includes bariatric program development, patient and staff education, as well as the oversight of multidisciplinary teams and acting as! the liaison between the hospital and surgical practice.

Es! sential Duties and Responsibilities:
1. Assists in administrative functions and the development & implementation of patient care protocols, clinical pathways and in updating patient policies and procedures for care of bariatric surgery patients.

2. Makes daily rounds on all bariatric surgery patients. Assesses and monitors the continuity of care and expedites and coordinates the delivery of services to facilitate a patient's progress throughout the continuum of care, from point of first interest, through the qualification period, the surgery process, post-op care, and post discharge to achieve desired quality outcomes.

3. Assumes a leadership role to facilitate interdisciplinary collaboration and review the plan of care and communicate findings effectively with all members of the multidisciplinary team. Documents in the medical record and works in conjunction with the dietitian, physical therapist, wellness program coordinator and/or exercise physi! ologist to coordinate facility nutrition programs, exercise programs, or medical management programs (for both the surgical and non-surgical candidates) and informs patients of these programs. Maintains outpatient records accurately, completely, timely and confidentially according to policies.

4. Facilitates patient and family education, and the discharge process to promote continuity of care after discharge and optimal patient outcomes. Reviews and revises patient education materials. Performs telephone follow-up surveys on patients post discharge, at specified intervals and annually for up to 5 years.

5. Functions as a clinical liaison and resource for the hospital program. Is responsible for assisting the hospital with achieving and maintaining BSCOE certification/accreditation through meeting or exceeding COE standards of care and participating in site visits, tour of facility, evaluation of center and quality of care, interviews, random chart audits a! nd rectifying any identified issues.

6. Assists in the identif! ication and reporting of quality issues, and in the development of a performance improvement plan for the Bariatric Surgery Program to assess and improve the quality and efficiency of the program including but not limited to quality assessment, utilization review, risk management, and infection control. Identifies appropriate indicators to monitor and track and in conjunction with marketing, tracks patient data advertising campaigns, and patient referral outcomes.

7. Works in conjunction with and assists leadership and clinical nurse specialists indeveloping curriculum and providing skills labs or in-service education for Medical Staff, employees or contract staff regarding the special needs of bariatric patients. (Sensitivity training, lift & transfer techniques, complications of bariatric surgery, post-op care psychosocial needs etc.)

8. Assists and attends monthly community lectures and support group meetings, and keeps class advertising and calendar li! stings updated for the website and other advertising venues. Coordinates and collaborates with the patient support group facilitator and facilitates meetings as necessary in the absence of a facilitator. Facilitates and participates in the post-op patient care lectures including nutrition, exercise, and psycho-social changes.

9. Conducts pre-operative patient education classes.

10. Exhibits behaviors and actions which create a high level of patient/ customer satisfaction, which contributes to positive patient/customer relations and which reflect respect for the patient's/customer's rights, needs and confidentiality.

11. Demonstrates effective communication and human relation skills which promote harmony and teamwork.

12. Presents behavior and actions which maintain the hospital's credibility, integrity and positive image.

13. Demonstrates a commitment to the strategic objectives of AMC's weight loss programs

14. Function! s without direct supervision, utilizing time constructively and organiz! ing assignments for maximum productivity. Maintains acceptable standards in the areas of attendance, professionalism, and establishing harmonious working relationships.

15. Assumes responsibility for the safety of patient/customers and ensures all patient care is provided in accordance with Hospital and other regulatory requirements to include: JCAHO, OSHA, State, Federal, Infection Control, Patient Safety Goals etc.

16. Exhibits flexibility, adapting readily to changes in the work environment or work schedule.

17. Assumes responsibility for professional growth and development by staying current in the field by reading related professional literature and undergoing annual bariatric related training.

18. Maintains strict confidentiality of PHI (Personal Health Information) following HIPAA policy and government regulation. Maintains strict confidentiality of employee information.

Qualifications

:

Minimum Educat! ion and Training Required:
1. Registered Nurse in the State of Georgia

2. Bachelor's degree in Nursing.

3. Bariatric Nurse Certification

4. Current BCLS

5. Minimum three years experience in the management of a bariatric program includes proficiency in program development, insurance and marketing experience.

6. Approximately three to five years of clinical nursing experience, nursing management experience of quality management, case management or discharge planning experience.

Preferred Education and Training:
1. Master's degree in Nursing from an accredited college or university or equivalent experience.
